Lactation Station Florida LLC Payment Policies

Definitions

In this document, “Lactation Station Florida LLC” refers to Lactation Station Florida LLC and its designated International Board Certified Lactation Consultant(s) (“the IBCLC”).

Insurance Coverage / Self-Pay / Out-of-Network Services

Lactation Station Florida LLC is an in-network provider for Cigna through our partnership with Wildflower Health. For families with Cigna, coverage and billing will be processed through the Wildflower platform.

For all other insurance providers, we currently operate as an out-of-network provider on a self-pay basis. Payment is required for all services provided, including in-person visits, telehealth consultations, and any applicable travel fees for home visits. Travel fees are not eligible for insurance reimbursement.

Travel Fees for Home Visits

Travel fees may apply for in-home visits depending on the distance from our office (located at 1351 Alafaya Trail, Oviedo, FL 32765).

• Within approximately 10 miles (~0–20 minutes): Visits to Oviedo, Winter Springs, or Casselberry are typically included in the standard consultation fee, though these cities are provided as examples only and the fee may vary based on exact location.

• Approximately 10–20 miles (~20–40 minutes): Visits to nearby areas such as Sanford, Lake Mary, Winter Park, Altamonte Springs, or eastern Orlando may incur a travel fee of $25–$35, depending on distance and travel time.

• Beyond approximately 20 miles: Visits to locations farther than 20 miles may be accommodated with a travel fee starting at $50, evaluated on a case-by-case basis.

Travel fees are in addition to the standard consultation fee and are not eligible for insurance reimbursement. 

For the most accurate estimate, please check the distance from your residence to our office using Google Maps to see whether a travel fee may apply.

Important Notice! Our Oviedo office is currently preparing for its grand opening in February 2026. During this pre-opening phase, we are exclusively providing in-home and virtual consultations. To support our local families, travel fees are currently waived for all home visits within our service area until our physical location is officially open for clinical services.

Payment Authorization and Timing

At the time of scheduling, I understand that I will be required to provide a valid credit or debit card to be kept on file.

Payment for services is due at the conclusion of the consultation. If I prefer not to complete payment at that time, I authorize Lactation Station Florida LLC to automatically charge the card on file approximately two (2) hours after the consultation has been completed.

This authorization applies to all scheduled services, including telehealth and in-home visits, as well as any applicable travel fees, cancellation fees, or no-show fees, in accordance with these Payment Policies.

Payment Processing Platform

Payments may be processed electronically. Lactation Station Florida LLC currently utilizes PracticeQ Payments to securely store payment information and process charges in accordance with applicable security and privacy standards.

I understand that electronic payment processors may send receipts by email or text message that could include limited personal information, such as the date and type of visit. If I prefer not to receive electronic receipts, I may request an alternative payment method, such as cash or check, when available.

Superbills and Insurance Reimbursement

Upon request, Lactation Station Florida LLC will provide a superbill, which also serves as a payment receipt. The superbill will include appropriate diagnostic and procedure codes based on the level of service provided.

I understand that submission of the superbill to my insurance company is my responsibility. While some insurance plans may reimburse lactation services, reimbursement is not guaranteed and depends entirely on my individual insurance plan, benefits, and insurer policies.

Lactation Station Florida LLC does not guarantee reimbursement and is not responsible for denied, reduced, or delayed insurance payments.

Responsibility for Payment

I understand that I am fully financially responsible for all services provided, regardless of insurance coverage or reimbursement outcomes.

Lactation Station Florida LLC provides care to me and to my baby or babies, and these services collectively constitute the client relationship.

Complementary Services

Our consultations may include integrative therapeutic modalities, such as Red Light Therapy (Photobiomodulation), when clinically indicated by the IBCLC. These services are provided as a complementary part of your clinical care at no additional cost.

Follow-Up Support

My initial visit includes one (1) week of follow-up support via secure messaging, email, or text, as determined appropriate by the IBCLC. Continued support beyond this period may be available for an additional fee. These fees are elective and not eligible for insurance reimbursement.

Rescheduling

Clients may reschedule their appointment up to 24 hours in advance without any fee. We encourage families to contact us as soon as possible if they need to change their appointment time to avoid cancellation fees.

Cancellation and No-Show Policy

I understand that I am responsible for all charges associated with my appointment. All appointments canceled within 24 hours of the scheduled time will be charged 50% of the applicable rate. All no-shows will be charged 100% of the applicable rate. Examples:

Fees for cancellations or no-shows are not eligible for reimbursement by insurance. I understand that these fees will be charged to the card on file in accordance with my payment authorization.

General Provisions

Lactation Station Florida LLC may communicate with my credit card company or bank regarding payment-related matters. It is my responsibility to provide accurate and current payment information and to update this information as needed. I am responsible for any fees or costs resulting from failure to provide valid payment information.

These payment policies apply to Lactation Station Florida LLC and its representatives.
